Employee Personnel Files, Certificated POLICY NO: 431
Records Required
A personnel folder on each certificated employee will be maintained in the office of
the superintendent of schools. All certificated employees shall be responsible for
maintaining the following information in that file:
- A completed application for employment.
- A complete copy of the employee's college credits.
- A valid teaching certificate or a record of its endorsement for use in this school
district.
- A federal income tax withholding form (W-4).
- A social security number.
- An I-9 form for employees after October 1988.
- Any other information or records deemed essential by the board of trustees or its
agents.
